Membership Terms & Conditions
Eleven Wellness offers several memberships designed to provide consistent access to professional treatments along with exclusive member savings.
Membership benefits may include monthly treatments, preferred pricing on services, and additional member-only perks. Benefits are available only while the membership is active and in good standing.
Specific inclusions vary by membership tier.
Minimum Commitment
All memberships require a minimum three (3) month commitment.
Members agree to maintain active billing for three consecutive monthly payments. Early cancellation before completing the three-month commitment is not permitted.
After the initial term, memberships automatically continue on a month-to-month basis unless cancelled prior to the next billing date.
Billing & Automatic Payments
Membership dues are automatically charged to the payment method on file each month.
By enrolling in a membership, members authorize Eleven Wellness to process recurring monthly payments for membership dues and any approved service upgrades.
Members are responsible for maintaining valid payment information.
If payment fails:
- Membership benefits may be temporarily suspended
- Services may not be scheduled until payment is brought current
- Repeated failed payments may result in membership termination
Membership fees are non-refundable once billed.
Cancellation Policy
Memberships may be cancelled after the initial three-month commitment by providing written notice prior to the next billing date.
Cancellation will take effect at the end of the final paid billing cycle.
No refunds will be issued for partially used months or unused services.
Unused Services
Monthly treatments must be used within the active billing cycle unless otherwise stated.
If membership is cancelled:
- Members have 30 days from the final billing cycle to redeem any unused service.
- Services not used within this period will expire and be forfeited.
Unused services:
- Have no cash value
- Are non-refundable
- Cannot be exchanged for other services.
Transfers & Sharing
Membership benefits are intended for use by the enrolled member only and may not be shared unless otherwise specified by the membership plan.
Member Discounts
Active members receive exclusive savings on select services and retail products.
Member discounts:
- Apply only while membership is active
- Cannot be combined with other promotions unless otherwise stated
- May not be applied to previously purchased services
Medical & Treatment Eligibility
Certain treatments offered through membership programs may require medical consultation and provider approval.
Eleven Wellness reserves the right to modify, delay, or withhold services if a treatment is deemed medically inappropriate or unsafe.
Policy Changes
Eleven Wellness reserves the right to modify membership pricing, benefits, or policies with notice to members.
